How to Book an Appointment?
All patients must have a referral from a physician or healthcare provider before booking an appointment. Requisition forms can be printed off the website, received from your doctor, or picked up from one of our 3 locations. If one of these possibilities in unavailable to you, please speak to one of our receptionists for other arrangements.
Following your referral, you can call or visit the center for an appointment. We would be happy to give you a tour of our facility to ensure you will be comfortable, and familiarize you with the procedure and staff. If you have any questions or concerns you can email info@mykimaging.ca.
